Direct Care
Direct Care is a last resort medical equipment program for families in need. Variety reviews all applications on a monthly basis.
Direct Care funds provide the following equipment to families:
- Wheelchairs
- Walkers
- Strollers
- Sitters
- Standers
- Prosthesis
- Bathing Needs
- Hearing Aids
- Braces
- Adapted Tricycles
- Car Seats
- Helmets
- Educational and Recreational components
Direct Care does NOT fund:
- Ramps
- Vans
- Bed Rails
- Computers
- Utility Bills
- Home Repairs and/or Modifications
To be considered for medical equipment funding, you must submit a completed application (including the photo consent form) and a letter of denial from your insurance company.
In addition to this completed form, we also request a letter of denial from your insurance agency. Click Download Form at the bottom of the page to access the Direct Care application. Please send both to:
