New Registration Process for Variety at Play Events
In an effort to better reach our intended audience, Variety is now requiring an Intake Form be filled out, and put on file, for each child with a disability who registers for a Variety event. This form will allow Variety to verify your child’s disability. This form only needs to be filled out and submitted once*. Your child’s form will then be kept on file for future events. Only children with a verified disability will be admitted to our events.

The first two pages can be filled out by the child’s parent/guardian. The third page MUST be filled out, and signed, by a licensed practitioner.
Completed forms can be mailed to Variety’s main office at:
1520 Locust Street, 2nd Floor, Philadelphia, PA 19102
or faxed to (215) 735-2450.

*Please note that filling out the Intake Form does not automatically register your child for our events. You still must register for each event that you and your child wish to attend.
